Dear ICDR Working Group members,
My name is Taryn Bolt, and I am a second year occupational therapy student
conducting a study to evaluate research interests and abilities of ICDR
Working Group members.
You are receiving this email because you are a member of one of the ICDR
working groups and we would like to invite you to complete this survey by

As the working groups are primarily comprised of volunteers I realize that
this survey is one of many demands on your time, and your participation in
the online survey is greatly appreciated.
*The survey should take approximately 10-15 minutes to complete.*
Participation in this survey is completely voluntary. If you choose to
participate please read the information below.
* Risks & Benefits:*
Please note that participation in this online survey does not pose any risks
to you. However, your input has the potential to shape the development of
the ICDR, as the results of this study may help the ICDR utilize its
resources more effectively, and increase its research output.
* Presentation of Findings:*
The findings from this study will be presented at the annual Thelma Cardwell
Research Day, which is held by the second year students from the department
of Occupational Therapy and Occupational Science. All participants and
interested parties will be invited to attend, and alternately a summary of
the results will be provided to all interested participants.
* Consent:*
The submission of a completed survey will constitute consenting to
participate in this study. Should you decide for any reason not to continue
with the survey you are free to do so without penalty.
Finally, your decision to participate or decline to participate in this
study will have no bearing on your working relationship with ICDR or any of
its members.
*Confidentiality:*
Please be assured that your identity will not be available to the researcher
analyzing the data, as the survey monkey tool allows for an anonymous
collection of responses. Subjects will not be identified by name in any
reports of the completed study. Finally, surveys will be destroyed after the
data analysis is complete, and the results have been presented at the annual
Thelma Cardwell Research Day.
* Remuneration/Compensation:*
Please note that there will be no remuneration or compensation for
participation in this research as it is a student run project.
